1. Service Description

FixMyDenial ("the Service") is a document preparation service that helps consumers draft appeal letters in response to health insurance claim denials. The Service uses professional analysis tools to identify denial reasons and generate appeal letter drafts, which are then reviewed by our team before delivery.

2. Not Legal or Medical Advice

The Service does not provide legal advice, medical advice, or any form of professional counsel. The analysis, recommendations, and appeal letters generated by the Service are informational tools designed to assist you in understanding and responding to insurance claim denials. They do not constitute legal opinions, legal representation, or a guarantee of any specific outcome.

You should consult a qualified attorney, patient advocate, or healthcare professional for advice specific to your situation. No attorney-client relationship is created by using this Service.

3. No Guarantee of Results

While we strive to provide accurate analysis and effective appeal letters, we cannot guarantee that any appeal will be successful. Success rates cited on our website are based on general industry statistics and do not guarantee your individual outcome. Results vary depending on the specific circumstances of each claim, insurer policies, and applicable regulations.

4. User Responsibilities

By using the Service, you agree that:

  • You are providing accurate and complete information about your insurance claim denial.
  • You will review any generated appeal letter carefully before authorizing its submission.
  • You understand that the appeal letter is a draft proposal that you may edit before signing.
  • You are authorized to submit an appeal for the claim in question.
  • You will not use the Service for fraudulent or unlawful purposes.

5. Pricing and Payments

The initial denial analysis is provided free of charge. The standard fee for appeal letter generation and submission is $49 per case. During promotional periods, this fee may be waived entirely. Payment, when applicable, is processed securely through Stripe. All fees are non-refundable once the appeal letter has been generated, unless otherwise required by applicable law.

6. Privacy and Data

We collect personal information necessary to prepare your appeal letter, including your name, email address, insurance policy details, and the content of your denial letter. This information is stored securely and is only used for the purpose of providing the Service. We do not sell your personal information to third parties.

Your denial letter and personal information may be processed by our analysis systems to generate the analysis and appeal letter. By using the Service, you consent to this processing.

7. Digital Signature

By providing a digital signature through the Service, you confirm that you have reviewed the appeal letter in its entirety, made any desired modifications, and authorize FixMyDenial to submit the letter on your behalf to your insurance company via email, fax, and/or postal mail.

8. Limitation of Liability

To the fullest extent permitted by law, FixMyDenial shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to your use of the Service. Our total liability for any claim arising from the Service shall not exceed the amount you paid for the specific case in question.

9. Changes to Terms

We reserve the right to update these Terms of Service at any time. Changes will be effective upon posting to this page. Your continued use of the Service after changes are posted constitutes acceptance of the revised terms.
